Required Skills
- Interprets plans, establishes, and exercises proper treatment program in accordance with the physician’s referral, periodically, reviewing the patient’s program and response and making recommendations to the physician relative to the need of continuing or discontinuing the physical therapy treatment
- Assists the physician in evaluating disability, injury, or disease process of individual patients and in evaluating the patient’s level of function
- Contacts the physician directly for patient reports and care as needed
- Observes, records, and reports to the physician the patient’s reaction to the therapy program or any changes in the patient’s condition
- Submits to the physician appropriate written reports to the patient’s progress at the designated intervals using appropriate and designated forms
- Assists in determining the needs of the patient and in developing the total plan of care
- Treats patients and their families with respect and dignity
- Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families/care givers
- The clinician will need to demonstrate the knowledge and skills necessary to provide care appropriate to ages specific to the patient population for the clinician. This could include the following groups: infants (0-12months), children (1 year – 12 years), adolescents (13 years -18 years), adult (18 years-65 years), older adults (65 years +).
- Demonstrates the knowledge and skills necessary to provide care appropriate to infants (0-12months), children (1 year – 12 years), adolescents (13 years -18 years), adult (18 years-65 years), older adults (65 years +)
- Instructs, teaches and supervises patients regarding physical therapy procedures as related to the patient’s therapy programs
- Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family and care givers are included in teaching as appropriate
- May supervise physical therapist assistants, rehabilitation aides, volunteers, or students in regards to patient care techniques and needs
- Enhance professional development through participation in continuing education programs and in-service meetings
- Attend and participates in meetings as required
- Adheres to all applicable federal, state, local, and company maintained standards of care and ethics and policies concerned with the practice of physical therapy and among other things, personnel qualifications
- Meets productivity and utilization expectations as established by the facility
- Completes timely clinical documentation to include functional goal-setting, progress and therapeutic approach as well as administrative/billing documentation timely and accurately
- Assists in maintaining department policies, procedures, objectives, quality assurance, safety, environmental, and infection control programs
- Assist in care and maintenance of department equipment and supplies
- Performs other related work as required or requested.

Required Experience
Licensure/Certification/Registry:
- Illinois licensed/licensure eligible as a Physical Therapist.
- Certified in CPR or complete certification within 30 days of employment.

- Medical-surgical or post-surgical nursing experience preferred
- Proficiency in wound care, IV therapy and complex medication administration
Key Responsibilities:
- Administer medications and IV therapies as prescribed.
- Monitor dialysis treatments and manage patients with complex conditions.
- Work with LPNs, CNAs and interdisciplinary teams to coordinate patient care.
- Ensure accurate documentation and compliance with hospital protocols.
- Assist in post-operative recovery, monitoring patients and managing complications as needed.
Required Experience
Education, Licensures & Certifications:
- Active RN license in Illinois required
- Current BLS/CPR certification required within 30 days of hire
- ACLS certification required within one year of hire
- BSN preferred but not required

Position Summary
Memorial Health is a community-based, not for profit health system that provides a full range of inpatient, outpatient, home health, hospice, primary and specialty care and urgent care physician services throughout central Illinois. Our providers serve both urban and rural communities at clinics located throughout a multi-county area. Employing nearly 300 primary, specialty and urgent care providers, Memorial is a leader for patient care services in the area. Position Details:
• Monday – Friday
• Flexibility on hours/day worked
• Outpatient only
• Call rotation – Oncall varies depending on location and clinic partner pool. Telenurse provides after-hours triaging for the physicians. Telenurse triages and follows set protocols to reduce the volume of phone calls/pages the physicians receive after-hours. Call volume is minimalized with triage services.
• Clinic visits: 18-22 per day
Position Requirments:
- Unrestrictive Illinois license prior to start
- Board Certified or Board Eligibility
- Dual Board certified
- 2+ years of prior practicing experience is preferred, but not required
