Greets patients/family members and offers assistance.

Completes all steps of pre-registration/registration including patient interview, collects insurance information, obtains photo identification and signatures for consent and distributes required patient information.

Coordinates accurate patient data collection to verify insurance eligibility and determines financial obligation, which may include collection of copayments.

Ensures compliance with all applicable HIPAA, Joint Commission, CDC, TMH and state and federal statues, providing required associated literature to patients at all PAS access points.

Responds to all emergency procedures.

Pushes/Assists patients in wheelchairs to the location of the service they need.

Communicates with co-workers and Manager on follow-up or concerns.

Demonstrates an ability to be flexible, organized and function well in a stressful situation.

• Daily direct contact with patients and customers – excellent interpersonal skills required.
• Accesses computer terminal to input, update, retrieve and review patient admission information on new and established patients.
• Answers phones and takes accurate messages.
• Confirms orders by daily contact with referral sources, attains proper patient information, written physician order and preauthorization of insurance.

• Medical Records

• Establishes, prepares and files outpatient medical record charts for all services
• Copies, files, faxes, scans and routes outpatient medical records and correspondence to physicians, medical records department and third party payers.
• Pulls medical record charts daily for all service areas.

• Billing

• Checks billing sheets daily for accuracy and complete information
• Inputs patient data, charges and adjustments on a daily basis
• Generates statistical billing data s requested by manager/director.

• Rehab Support

• Assists with rehabilitative services for patients of all ages under the direct supervision of the therapists and/or assistant.
• Transports patients to and from therapy in accordance with patient schedule.
• Prepares the patient and treatment area prior to and following treatment procedures.
• Maintains inventory of rehabilitation equipment and assists for ordering of supplies.
• Provides coverage for other staff when needed.
• Demonstrates support for and participates as requested in accomplishing Rehab Services goals and objectives.

• Schedules all appointments and maintains efficient Master schedule for OT/PT/SLP. Keeps manager informed of concerns regarding wait times.

• Answers phone and greets customers in a friendly manner.

• Registers patients, verifies physician’s orders, updates demographics and insurance on existing patients and obtains signature for proper billing.

• Pre-certifies patients insurance when needed to ensure payment and correct billing.

• Completes discharge process on all patients. Forwards appropriate record information to Health Information.

• Participates in Competency Assessment Process.

Required Skills

1. Responsible for daily occupied and discharge/transfer cleaning and disinfection of patient care areas, equipment as assigned and other common areas of assignment.
2. Performs duties while demonstrating working knowledge of the health care cleaning and disinfecting principles and requirements, proper and safe chemical use, equipment handling, and labor saving products for all items within the healthcare environment for which tasked with servicing.
3. Adheres to all organization safety, risk management and infection control guidelines and standards while promoting a culture of safety.
4. Actively participates in departmental performance improvement initiatives, including but not limited to initiatives to improve patient experience and cleanliness metrics.
5. Maintains cleaning cart(s), supplies and equipment evidence by properly stocking and restocking environmental services cart in a neat and orderly fashion.
6. Removes waste and transports to the appropriate disposal area while identifying proper waste streams handling (i.e. regulated medical, recycle, solid, and hazardous).
7. Follows all infection prevention practices (i.e. proper cleaning and disinfection protocols and process, hand hygiene, standard & isolation precautions, as well as proper donning and doffing of Personal Protective Equipment (PPE)
8. May be asked to perform specialty tasks according to facility needs including but not limited to tasks such as conference room set up/dismantle according to function sheets and schedules, cleaning of stairwells and elevators, stocking EVS supply rooms and hanging of cubicle curtains.

Required Skills

Direct Clinical Services

1. Develop initial comprehensive assessment data including clients presenting problems, mental status, relevant psychiatric, medical and developmental histories to make decisions regarding diagnosis (per DSM-V), treatment objectives and ongoing treatment plans and/or to provide information and make appropriate referrals to other treatment resources in the community.
2. Provide 25 hours of billable services per week to clients.
3. Implement crisis or pre-crisis intervention procedures with potentially suicidal, homicidal, or severely disabled clients. Where appropriate referrals will be made to consulting psychiatrists or inpatient psychiatric facilities to prevent further destabilization.
4. Interview collateral contacts, previous and current treatment professionals, and significant others, to further establish a database for assessment and ongoing treatment plan as needed.
5. Clarify priority problems, goals for change, and session limits according to agency policy to assist clients in developing appropriate expectations for treatment in relationship to the agency. Use a variety of psychotherapeutic treatment approaches and interventions to change targeted behaviors and stabilize adaptive behaviors with individuals, couples and families.
6. Manage the termination process and discharge planning to enhance clients’ abilities to retain treatment changes and/or use other available support services.
7. May organize, plan and participate in therapy groups for clients and/or their families, as assigned.

Client Information System

1. Establish appropriate professional files, including: comprehensive assessments, progress notes, treatment objectives and plans, previous treatment records, clients’ consent to current treatment and for exchange of information.
2. Complete required administrative forms. Collect necessary information for billing and fee collection for both community mental health and private pay clients.
3. Provide necessary client information to other agencies or health care providers to coordinate services to identified clients.
4. May prepare necessary documents/reports for DCFS as needed.
