Position Summary

Full-Time
Evening Shift

Under the guidance and supervision of a licensed Pharmacist(s) and following all Federal and State regulations, prepares and distributes medications and maintains drug inventory and patient records.  Performs specialty functions in support of medication reconciliation. Obtains a current medication list for admitted patients through in-person interviews. Performs second sourcing by collecting medication list and verifying this with a second source.

Required Skills

  1. Conduct scripted interviews with patients admitted or to be admitted to the hospital.
  1. Obtain the patient’s most accurate and current home medication list, including prescription and non-prescription medications.
  1. Obtain the patient’s medication allergy history.
  1. Resolve home medication list discrepancies with an inpatient hospital pharmacist and/or a MH approved second source of information.
  1. Accurately document the home medication list and allergy information in the electronic health record

  1. Provides training for lesser-experienced colleagues as needed.
  1. Serves as a back-up for other areas within the unit or department, especially during times of special needs or colleague absences

Required Experience

Education:

Education equivalent to graduation from high school or GED is required.

Licensure/Certification/Registry:

  • Possesses a current State of Illinois Technician license, with Certified designation if required by the Illinois Pharmacy Practice Act. Applicants may not begin work without proof of licensure and certification (if required).  
  • Pharmacy Technician Certification through the Pharmacy Technician Certification Board-(PTCB) or the Exam for the Certification of Pharmacy Technicians (ExCPT) required with the following exceptions. If the colleague has one of the exceptions, the certification is highly preferred.
    • Pharmacy technicians who obtained initial licensure prior to January 1, 2008
    • Pharmacy students enrolled in an accredited school of pharmacy
  • Pharmacy Technician Certification Board (PTCB) Medication History Certificate (or equivalent) required within 12 months of hire/transfer after 01/2022.
    • If hired/transferred prior to 01/2022, the Medication History Certificate is highly preferred.

Experience:

  • Minimum 2 years of experience in retail/community pharmacy practice or 5 years hospital pharmacy experience required. Current enrollment in the final year of a Doctor of Pharmacy program will be considered in lieu of pharmacy experience.

Other Knowledge/Skills/Abilities:

  • Demonstrates excellent verbal and written communication skills, interpersonal skills, and customer relations skills
  • Demonstrates excellent knowledge of brand and generic medication names, common doses and dosage forms, and common administration frequencies
  • Ability to accurately and efficiently input data into the electronic health record, and effectively utilize drug information resources
  • Maintain confidentiality of patient information
  • Ability to set priorities
  • Leads by example
  • Demonstrates commitment to safety in all elements of daily practice.
  • Ability to push, pull, and transport up to 50 lbs. and stand/walk for long periods of time
